- What is a Geriatric Care Manager? A Geriatric Care Manager (GCM) is a professional
health and human service specialist who develops and implements a plan to assist elders and their families with all aspects
of long-term care. The GCM is trained and experienced in any of several fields related to long-term care including,
but not limited to, nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging
and elder care.
- Why haven't I heard of a Geriatric Care Manager?
Geriatric Care Management is a relatively new field of expertise designed to address the needs of an increasing senior population.
Specialized services are available for seniors and a GCM has an established network of specialists and care providers who
can offer the services a senior needs.
- How can a Geriatric Care Manager help me?
If you have any elderly parent, spouse, friend or other loved one who is experiencing a sudden medical issue, change in behavior,
etc., a GCM will conduct a thorough in-home assessment, develop a care plan (a written plan of action to address the needs
identified in the assessment), and recommend specialists and care providers such as doctors, lawyers, accountants, financial
advisors, assistant living and long-term care facilities, and in-home services, etc. You decide whether to use one of
our care providers who you can select you own and we will coordinate the services.
- What
are the benefits of using a Geriatric Care Manager? Because a GCM has specialized knowledge and experience
in senior care issues, we can save you time and money. Senior
Living has a list of specialists and service providers such as geriatricians, neurologists,
cardiologists, eldercare lawyers, financial planners, in-home care providers, etc., at our fingertips. This will save
you time and the inconvenience of locating a provider. Senior
Living can also avoid duplication of services, unnecessary hospital stays and inappropriate
placement in alternative living facilities that can be costly. We will coordinate the services and provide
communication between the family, doctors and other professionals, and care providers. Senior Living becomes an integral part of the family to provide personalized
and compassionate services.
- What does it cost for a Geriatric Care Manager's
services? At Senior Living we
are sensitive to our client's needs and will work with you to develop a care plan that would not drain your wallet. Because
each client's needs are different, the cost for their care may vary. Senior
Living will provide a free 30 minute telephone consultation to discuss your loved one's situation. If
you decide our services are needed, we will schedule an in-home assessment that could take several hours. A written care
plan will be developed with recommendations. You will decide what services you can afford. Senior Living does not receive any commission
or referral fees from specialists or service providers. We will arrange the services and you will pay the provider directly.
- What assurances do we have that your specialists and service providers provide quality
service? Senior Living will
not use any provider that does not meet its quality standards. The physicians, specialists and service providers are
"hand picked." We recommend care and service providers who we know provide quality care and service.
It is important that you realize that Senior Living only
recommends these providers. You can agree to use one of ours or you can select one of your own. We do not have
any contracts or other business arrangements with these care and service providers. We will coordinate the services and you
will pay the provider directly.
- What areas do you serve? Senior Living primarily serves Howard, Baltimore and Anne Arundel Counties
in the Maryland area.
